On October 18, the general office of the national health and Family Planning Commission publicly issued the notice on printing and distributing the guidelines for medical service capacity of tertiary general hospitals (2016 version) (gwbmyh [2016] No. 936).


After soliciting opinions for two years, the health and Family Planning Commission's Guide to the medical service capacity of tertiary general hospitals (2016 Edition) (hereinafter referred to as the guide) was finally released, which also set a clear yardstick for the appearance of tertiary general hospitals nationwide in the future.
According to the guidelines, the tertiary general hospital should have medical equipment that matches the service functions of tertiary hospitals and can meet the needs of clinical diagnosis and treatment of difficult and severe cases, as well as medical technology and other services.


The necessary medical equipment of level III General Hospital includes:
☑  Automatic biochemical analyzer
☑  Automatic chemiluminescence immunoanalyzer
☑  Color Doppler ultrasound (2D color Doppler ultrasound, 3D color Doppler ultrasound)
☑  Computed tomography (CT) (64 rows and above)
☑  Digital X-ray (DR, Cr)
☑  Magnetic resonance imaging (MRI) (1.5T and above)
☑  Digital subtraction angiography (DSA)
☑  Digital gastrointestinal fluoroscopy machine
☑  Breast X-ray machine
☑  Gastrointestinal X-ray machine
☑  Mobile C-arm digital subtraction angiography machine, etc.


In addition, the tertiary general hospital should also have appropriate bed size, diagnosis and treatment subjects and health technicians with reasonable structure, so as to meet the service function, technical level and management requirements of the tertiary hospital.


In terms of bed size, the proportion of surgical beds to the actual number of open beds in the hospital should be ≥ 30%. The number of beds in the intensive care department (including all professional ICUs) should account for 2% - 8% of the number of beds actually opened in the hospital.


As for the specific diagnosis and treatment subjects, the tertiary general hospital shall set up but not limited to the following diagnosis and treatment subjects:


1. Primary diagnosis and treatment subjects


The primary diagnosis and treatment subjects that should be established include: prevention and health care department, internal medicine, surgery, gynecology and obstetrics, pediatrics, ophthalmology, otolaryngology, Stomatology, dermatology, psychiatry, infection department, oncology department, emergency medicine department, rehabilitation medicine department, anesthesiology department, critical medicine department, medical laboratory department, pathology department, medical imaging department and traditional Chinese medicine department.
Primary diagnosis and treatment subject that can be set: pediatric surgery.


2. Secondary diagnosis and treatment subjects


Internal medicine should include: Respiratory Medicine, digestive medicine, neurology, cardiovascular medicine, hematology, Nephrology, endocrinology and immunology. It can provide specialized medical services corresponding to geriatrics.
Surgery should include general surgery, orthopedics, neurosurgery, urology and thoracic surgery. There can be: cardiovascular surgery, plastic surgery, burn department.


Gynecology and obstetrics should include gynecology and obstetrics. There can be: family planning department, reproductive health and infertility department, eugenics department, gynecological endocrinology department.


Pediatrics shall provide specialized medical services corresponding to neonatal pediatrics. There can be: pediatric infectious diseases department, pediatric digestive department, pediatric respiratory department, pediatric cardiology department, pediatric nephrology department, pediatric hematology department, pediatric neurology department, pediatric endocrinology department, pediatric genetics department and pediatric immunization department.


Psychiatry should include psychosomatic medicine and psychotherapy. There can be: geriatric psychiatry, mental rehabilitation, children and adolescents psychiatry, sleep medicine.


The medical laboratory department shall include: clinical humoral test, blood test, clinical microbiology test, clinical biochemical test, clinical immunity and serology test, and clinical cytomolecular genetics test.


The medical imaging department shall provide professional medical services corresponding to the following secondary diagnosis and treatment subjects: X-ray diagnosis, computed tomography (CT) diagnosis, magnetic resonance imaging (MRI) diagnosis, nuclear medicine diagnosis and treatment, ultrasonic diagnosis, ECG diagnosis, EEG and cerebral blood flow diagram diagnosis, neuromuscular electrogram diagnosis, interventional radiology diagnosis and treatment, radiotherapy.
